EASy IVC

Pre-Induction Optimization Protocol

EASy IVC combines IVC assessment with the hemodynamic cluster framework to guide pre-induction optimization of critically ill patients — targeting volume and vasopressor management before induction.

Protocol Components

IVC diameter measurement (2cm from RA junction)
IVC collapsibility index calculation
Hemodynamic cluster identification (1-4)
Pre-induction fluid optimization targets
Vasopressor/inotrope recommendations by cluster
Post-induction reassessment protocol

IVC-Guided Optimization

Small (<1.5cm), collapsing

Volume resuscitate before induction

High risk of post-induction hypotension

Normal (1.5-2.5cm), partial collapse

Small fluid bolus, prepare vasopressor

Moderate risk

Plethoric (>2.5cm), no collapse

Avoid fluid, assess RV/LV function

May indicate cardiogenic or obstructive cause
